How to Report a Claim

We understand that the loss of a loved one can be a very difficult and confusing time. That’s why we make it our priority to keep the claims process as simple and streamlined as possible. To report a life insurance claim, just complete the steps below. Or call 1-800-695-5165

You should also have your relatives, friends, and neighbors call 1-800-695-5165 whenever THEY need to submit a claim.

1. Request a Claims Packet

instructions and forms needed to begin processing your claim.

By Phone

To request a claims packet by phone, call our dedicated claims line at 1-800-695-5165. Our business hours are
8 a.m. to 10 p.m. (Eastern Time) Monday through Friday, 9 a.m. to 5 p.m. Saturday.

Please have the following information available when you call: Contract Number or Social Security Number of insured, Date of Death, and Cause of Death.

2. Complete and Mail Your Claim Form

Mail your completed claim form and a certified death certificate (copies are not accepted) to:

3. Provide Additional Documentation If Needed

If necessary, a claims representative may contact you to request additional documentation needed to complete your claim. For a complete list, see the Claims Checklist below. These forms, if requested, can be mailed to one of the addresses above.

Here is a list of documents you will need to submit in order for us to consider your claim:
1. Completed claim form
2. Certified death certificate (copies not accepted)

The following documents may be requested depending on your specific situation:
3. Proof of guardianship for a minor beneficiary
4. Executorship papers for the estate
5. Funeral Home assignment(s)
6. Proof of accidental death (i.e. newspaper article, police report, etc.)
7. Survivorship Affidavit (also available in Spanish)
8. Foreign Death Questionnaire
9. Evidence of Age
